How can you review and correct Personal Information that we have about you?

Within 30 business days of receipt of your written request, we will describe the personal information we maintain, any parties with whom we have shared it within the last 2 years, and how you may request a correction, if necessary. You may receive a copy of it for a fee to cover our costs. We will not provide information that we feel is privileged, such as information about insurance claims or lawsuits.
To request a correction of information about you, send a written request as described above, explaining the desired correction. Within 30 business days, we will either make the requested correction or tell you why we will not. We cannot correct Consumer Report Information. To do this, you must contact the consumer reporting agencies that provided it. If we make the requested correction, we will notify you in writing and we will also notify anyone (i) who may have given or furnished the original information; (ii) who you designate who may have received the original information within the two years before the date we made the correction; and (iii) any insurance-support organization whose primary source of personal information is insurance institution if that organization has systemically received such information from us within the preceding seven years and continues to maintain information about you. If we do not agree, you may submit a written statement as to why you disagree, which will be saved in your file. We will send it to the same persons to whom we would send a copy of any correction or change.
Other Important Information
Arizona, Georgia, Maine, Massachusetts, Minnesota, Montana, New Jersey, North Carolina, Ohio, Oregon, Virginia Residents: This is notice that the information obtained from a report prepared by an insurance support organization may be retained by the insurance support organization and disclosed to other persons.
California Residents:Right to Correct, Amend, or Delete
In addition to the rights identified above in the section entitled"How can you review and correct Personal Information that we have about you?", if we refuse to correct, amend, or delete disputed personal information, you may file an appeal with the California Department of Insurance.
Massachusetts Residents:Right to Correct, Amend, or Delete
In addition to the rights identified above in the section entitled "How can you review and correct Personal Information that we have about you?", if we refuse to correct, amend, or delete disputed personal information, you may file an appeal with the Massachusetts Commissioner of Insurance.

In addition to the rights identified above in the section entitled "How can you review and correct Personal Information that we have about you?", if we refuse to correct, amend, or delete disputed personal information, the Montana Commissioner of Insurance may review the disputed personal information to determine if the information is correct
Montana Residents: Right to Receive Disclosures of Medical Records
Pursuant to Montana Code Ann. §33-19-202(4)(c), if you are a resident of the State of Montana, you have the right to receive, upon written request addressed to a licensee, a record of any disclosures of medical record information made by a licensee. Under Montana Code Ann §33-19-301, such records of disclosures must include the identity of any persons receiving medical record information during the preceding 2 years, the date of receipt of such information, and a description of the information.
Nevada Residents: We are providing you this notice pursuant to Nevada law.
